VPN Access Request Form

This form is required for staff, faculty, and authorized third parties to request Virtual Private Network (VPN) access via Palo Alto’s GlobalProtect. VPN access enables secure connection to institutional systems and resources from off-campus locations.

All requests will be reviewed and must be approved by your supervisor and IT Services. Approved users are required to read and agree to the terms of the Remote Access Policy.

VPN access is only permitted on devices running a supported, up-to-date operating system (OS).
Supported OS:

  • Mac: Sequoia, Sonoma
  • Windows: Windows 10 Workstation Pro, Windows 11 Pro
    • Note: Windows 10 Home and Windows 11 Home are NOT supported.
