Report Lost/Stolen Device

Provide information to report lost or stolen SMC technology.

Required

Police Report
You will be asked to attach a copy of the police report. If you cannot attach a police report at this time, make sure to add the report to the ticket as soon as possible. The police report is needed for IT Services to properly process your request and avoid charging your department unnecessary replacement fees. Contact the IT Purchasing department with any questions.

PPI/SPI
You will be asked if the device contained Personal Identifiable Information (PII) or Sensitive Personal Information (SPI) related to Students, Staff, Faulty, or any other Saint Mary's College affiliated individuals. Read below and be ready to answer this question when you submit the request.
 

PII and SPII as defined by the National Institute of Standards and Technology (NIST) Publication 800-122 are as follows. Examples of PII include, but are not limited to:

  • Name, such as full name, maiden name, mother's maiden name, or alias
  • Personal identification number, such as:
    • social security number (SSN),
    • passport number,
    • driver's license number,
    • taxpayer identification number, or
    • financial account or credit card number
  • Address information, such as street address or email address
  • Personal characteristics, including:
    • photographic image (especially of face or other identifying characteristic),
    • fingerprints, handwriting, or other biometric data (e.g., retina scan, voice signature, facial geometry)
  • Information about an individual that is linked or linkable to one of the above (e.g., date of birth, place of birth, race, religion, weight, activities, geographical indicators, employment information, medical information, education information, financial information).
